Information on Completing W9 Forms:

NESSP must report stipends paid to the educator to the IRS. Please download a blank W9 form from the IRS website:
W9 IRS Form.

The following fields are required:

  • Full name
  • Current address (where the check will be mailed)
  • Social security number
  • Signature (Do NOT type it in. Use a digital pen or print and sign.)
  • Date

Please make sure your W9 form is complete. We cannot send stipend checks to educators who submit an incomplete W9 form.
